MINDSET TO MOVEMENT CLIENT AGREEMENT
1:1 Coaching Agreement
1) INTRODUCTION
Congratulations! I'm excited to have you on board and a part of the Mindset to Movement Coaching Program. I cannot wait to work with you and support you to reach your goals.
Please read the following agreement carefully so that you know exactly what to expect. If you agree to the terms below, sign at the bottom.
2) WHAT YOUR PROGRAM INCLUDES
Your coaching package includes:
Access to me 7 days a week via the app
Personalised macros
Personalised movement goals
Weekly online check in forms
1 x zoom call at the beginning of the program
Nutrition support and advice
8 weeks of tailored nutrition, movement and habit support
As a client your program requires you to enter in and specifically choose your food that you eat. I am here as a guide and additional support to educate you how to make the correct choices for your nutrition and movement habits.
If your goal is weight loss the APP My Fitness Pal is used as an initial tool to identify your calorie intake to see if and what changes need to be made. If this is something that you do not wish to participate in then you can let me know and we can attempt to keep your calories low without that specific measure but please note results may not be the same.
3) RESULTS
Results are reliant on compliance to nutrition goals and movement goals. You agree to give 100% to this coaching relationship, knowing that you are ultimately responsible for your end results. I do not take responsibility for clients that do not adhere to tasks set thus not getting results. Your return of results in this program is determined by your level of commitment and effort which are factors outside of my control. Results are not a quick thing. My Program, as discussed in our consult is not designed to see a rapid weight loss in a short time frame so if this is what you are after then please do not sign the consent form.
If you ever feel like you are not getting what you have paid for then please make contact with me to discuss this. I cannot fix a problem if I do not know about it.
4) YOUR PAYMENT DETAILS
You’ve agreed to pay for your coaching on a recurring, weekly basis for 12 weeks minimum
Payment will be deducted weekly.
Thank you for this investment in our program and your health and wellness.
5) INTELLECTUAL PROPERTY RIGHTS AND ATTRIBUTION
All program materials remain sole property of Mindset to Movement. As a client you are not licensed to forward these documents to other parties or reproduce any of my materials without my consent.
6) NUTRITIONAL COUNSELLING/COACHING - LIABILITY RELEASE
The content of this nutritional program and any supplementary advice that accompanies it is to
serve as a general overview of matters of interest and is not intended to be comprehensive, nor
does it constitute medical (or other) advice in any way. This nutritional program is a compilation of
one person’s processes, concepts, philosophies, methodology and opinions. You should carry out
your own research and/or seek your own professional advice before acting or relying on any of the information displayed in your nutritional program whether it be in person or via online coaching.
The company, coach and its related entities will not be liable for any injuries, loss or damage that
may arise out of your improper use of, or reliance on, the content of this program. You accept sole responsibility for the outcomes if you choose to adopt and/or use the systems, methods, ideas, concepts and opinions within the content of this nutritional program.
You understand that Mindset To Movement is not a certified nutritionist and as an individual, your dietary needs and restrictions are unique. By signing this form, you acknowledge that Mindset To Movement is not a medical practitioner and any guidance or advice from Mindset To Movement does not constitute as medical advice. You also acknowledge that you have been advised to consult a physician before undergoing any dietary or food supplementary changes. You also affirmatively state that You have disclosed any and all known medical or genetic conditions, medications you use whether prescribed or not, and any significant personal or family medical history. Any recommendations that you follow for changes in diet, including
but not limited to the use of food supplements, are entirely my decision and my responsibility. You are knowingly assuming any risk associated with nutritional programming.
You have carefully read this waiver and understand it to be a release of all claims and causes of action for any injury or illness as a result of, or whilst participating in nutritional counselling or coaching.
7) CONCLUSION
Working with you is a privilege. There are many online coaches out there to hire and many ways to invest your time. I truly am honoured that you chose to work with me.
